Abstract: Balloon angioplasty, though effective for vascular stenosis, inevitably induces vascular injury during dilation. The absence of clinical interventions to promote endothelial regeneration post-injury predisposes patients to late-phase complications including thrombosis, inflammation, and restenosis. Photobiomodulation (PBM) is a biological intervention that can have a regulatory effect. Red light as its primary wavelength shows significant therapeutic promise in cardiovascular and neuropsychiatric disorders, which is especially important in cases where medication cannot be used. In this study, we evaluated the effects of 640-nm PBM at various doses on endothelial cells (ECs) and vascular smooth muscle cells (SMCs). In vivo, balloon-injured rat abdominal aortas underwent transvascular red light irradiation (20 mW/cm2, 3 min) via optical fibers. PBM promoted EC migration, proliferation, nitric oxide release, and CD31 expression while concurrently suppressing SMC proliferation and contractility. Rat model outcomes also confirmed PBM attenuated the vascular inflammation reaction and reduced neointimal hyperplasia. These findings show that PBM could be a viable strategy to enhance intima repair and mitigate balloon-associated vascular injury and restenosis.
