Abstract:
Background: Polyendocrine metabolic ovarian syndrome (PMOS), formerly known as polycystic ovary syndrome, is the most prevalent endocrine disorder among women of reproductive age. Cang-Fu-Dao-Tan formula (CFDTF), a traditional formula, has been reported to exert endocrine-metabolic regulatory effects in the treatment of PMOS. However, the effects and mechanisms by which CFDTF interrupts multigenerational PMOS-related features remain unknown. This study aimed to determine whether preconceptional CFDTF administration alleviates pathological phenotypes in the offspring of mothers with PMOS and to identify the potential underlying mechanisms. Methods: Prenatal anti-Mullerian hormone (PAMH)-induced PMOS mouse models (F0) were established, followed by preconceptional administration. Reproductive and glucolipid metabolic phenotypes were assessed in F1 and F2 offspring, and uterine transcriptomic profiling (RNA-seq) was performed in F0, F1, and F2 female offspring to investigate the potential mechanisms underlying CFDTF’s effects. Results: Preconceptional CFDTF administration significantly ameliorated reproductive abnormalities across generations. In F0-F2 females, it maintained estrous cyclicity, balanced testosterone levels, and reduced uterine inflammatory infiltration; in F1 males, it reduced abnormal seminiferous tubules. It also significantly improved glucolipid metabolism by lowering fasting glucose, total cholesterol, and hepatic lipid accumulation in F1 and F2 male and female offspring. RNA-seq analysis revealed multigenerational effects of preconceptional CFDTF administration on PMOS-associated uterine inflammatory signaling and immune microenvironment remodeling, which operate through a conserved cytokine network functionally linked to the interferon (IFN) inflammatory axis. Conclusions: Preconceptional CFDTF administration exerts positive multigenerational reproductive and glucolipid metabolic effects in a PMOS mouse model, through the alleviation of uterine inflammation.
